Pembina County Memorial
Hospital's Care Coordinator provides our patients and their
families with a complete line of discharge planning.
Do you need the help of a nurse or home health aide when you go
home? Or will you require some time in a nursing facility? Maybe
you need some home health equipment, a bed, walker, or other
appliance to make your home adaptable during your convalescence.
Our Care Coordinator will help you plan, and access community
resources as you prepare to leave the hospital. |

Our surgery department offers many different surgical
procedures that will help keep you close to home for all your pre and
post care needs. Among the most frequently performed surgeries at PCMH
are diagnostic laparoscopy, laparoscopic, orthopedic procedures, and
various general surgeries. Visiting specialists also perform a variety
of cataract and eye surgeries, tonsillectomies, and ear tube procedures |

a.m. - 6:00 p.m. Monday through Friday. On weekends the business office
is open from 7:30 a.m. - 3:30 p.m. on Saturdays and 9:00 a.m. -12:00 p.m.
on Sundays.

With 50 beds, Wedgewood
Manor is able to take care of your loved ones and provide all
the comforts of home. Our 24-hour skilled-nursing facility is
guided by a resident-directed care philosophy. Residents direct
their care to the highest extent possible. Therefore, assistance
is given only in areas where it is needed. Each staff member
works with the same residents on a regular basis to develop
trust, meaningful relationships and lasting bonds.
We have a wide variety of services available including skilled
nursing, medical care, physical, occupational and speech
therapy, rehabilitative and restorative programs, respite care, therapeutic recreation
and Adult Day Care.
Wedgewood Manor is a member of the North Dakota Long-Term Care
Association and the American Health Care Association.
Adult Day Care
Wedgewood Manor is now offering an adult day
care service. The intent of adult day care is to
provide older give care givers a break from their
daily responsibilities. Good candidates for the adult
day care program are seniors who:
• Can benefit from friendship and functional as-
sistance
• May be physically or cognitively challenged but
do not require 24 hour supervision
Are in the early stages of Alzheimer's Disease
The benefits of an adult day care include a secure
environment in which to spend the day, enjoyable
activities, improvement in physical and mental
health, an enhanced or maintained level of inde-
pendence, and nutritious meals and snacks.

PCMH's remodeled emergency room is newly designed to
better accommodate cardiac emergencies and ambulatory needs. The
emergency room is staffed 24-hours, seven days a week by medical and
nursing staff certified in advanced trauma life support and advanced
cardiac life support. PCMH is designated as a Level IV Trauma
Facility. |

Our Cardiac Rehab program offers
supervised exercise therapy and education to people who have experienced
a heart attack, heart surgery or heart disease.
Each part of the Cardiac Rehab program is designed to meet individual
needs. Treatment programs are developed based upon the patient's past
medical history, risk factor profile and current medical evaluation.
Each program is designed to help recondition or improve the function of
the heart muscle and to help the cardiac patient return to an active
lifestyle.
Participation in the Cardiac Rehab program requires a

At PCMH we offer area oncology patients the
opportunity to receive chemotherapy close to home in a much more relaxed
and family-like atmosphere. Intravenous therapy is administered in a
quiet and private environment following a treatment schedule ordered by
oncologists from Grand Forks and Fargo. |

The more patients know about managing their diabetes, the
more active they can be in their own care.
The Diabetes Education Center at PCMH provides diabetics of all ages
with a complete range of information on self-management training, blood
sugar monitoring, nutrition, exercise and more. The Center is designed
to take the time to answer the questions that your doctor might not
cover in a short office visit.
Doctor referrals are needed for all diabetic education services. For

treatment of their patients. |

PCMH
Physical Therapy department is devoted to treating, healing and
preventing injuries and disabilities for those we serve. We are
dedicated to excellence in patient care, the provision of a safe and
well-equipped work place and the operation of a cost-effective program.
Our licensed physical therapists work with patients to relieve pain and
discomfort, regain movement, promote healing, restore function, rebuild
confidence and motivation and help the patient adapt to physical
changes. We offer services for inpatients, outpatients and swing bed
patients at PCMH. We also supervise a restorative nursing program and
offer Medicare A skilled services at Wedgewood Manor.

There are highly qualified mobile units here every
week for your specific medical needs. Mobile units include CT, MRI, dexa
scan, ultrasound, mammography and nuclear medicine. |

PCMH in conjunction with Winmar Diagnostics, provides
obstructive sleep apnea diagnostic testing for adults, infants and
pediatric patients that may be experiencing respiratory related sleep
disorders.
A hospital study, involving sleep monitoring, may be done in an
overnight stay between 9 p.m. and 6 a.m. If you wish to be evaluated for
a sleeping disorder, contact your medical provider for evaluation. A
referral from your primary care physician is required for this program. |
